Position Description:
We are seeking a CNA/Adult Activities Assistant to join our team. This person is responsible for providing direct service to clients of the Specialized Adult Day Services (SADS), and must assist in all areas of care as specified by the care plan under the supervision of the nursing staff. This person reports to the Director of Brain Injury Community Services and/or the Registered Nurse on staff.

Key Responsibilities:
CNA related:
1. Assist with feeding clients as needed.
2. Take vital signs when necessary.
3. Documentation of all care provided.
4. Participation in client care activities.
5. Provide supervision of participants during operating hours of program.
6. Provide immediate response to emergency situations to assure the welfare of participants
7. Maintain positive interpersonal relations with clients, client family members and staff.
8. Report pertinent or needed changes in client condition or care plan to nurse or director.
9. Attend staff meetings and in-services as requested.
10. Assist in maintaining a clean professional working environment.
11. Provide a safe, clean therapeutic environment for clients.
12. Assess and report environmental hazards.
13. Facilitate bowel and bladder control.
14. Provide personal hygiene assistance.
15. Maintain personal hygiene and dress code standards.

Adult Activities Assistant related:
1. Facilitate, supervise and assist client participation in SADS activities.
2. Evaluate, assist and reinforce participants in the SADS.
3. Assist the Director in development and implementation SADS programs and activities.
4. Implement client plans as developed by the Director.
6. Provide immediate response to emergency situations to assure the welfare of participants
7. Complete and maintain daily logs of client activities and other documentation as necessary

• Previous experience working with persons with physical and/or intellectual disabilities.
• CPR and First Aid certification
• Certified Nursing Aide certificate issued by the Colorado State Board of Nursing.
• Two years of experience in an adult day service program or Home Health.

Physical Requirements
• Exert 25-50 lbs of force occasionally.
• Lift, carry, push, stand, walk, reach, sit, and crouch.
• Communicate effectively through speech, reading, and writing in the English language.
• Possess visual and auditory acuity.
