We are looking for a Health Services Director who has a passion for memory care and expertise working in assisted living or skilled nursing/long term care. This individual will be an integral part of the management team. The duties of this position include:
- Hiring, Management and scheduling of care provider staff
- Resident assessments
- Implementation and management of staff training program
- Resident care-plan development and review
- Management of medication administration program and supervising QMAP staff
- Hiring and Managing weekend/pt Wellness Manager
- Provide on-call support as needed
- Working directly with the Executive Director to implement the mission of the organization and with a mindset toward quality improvement


Requirements of this position include:
- Minimum of 3-years of experience working in memory care; assisted living, skilled nursing/long term care facility. Two years MUST be consecutive at one organization
- Ability to pass background check and drug screening
- Must have screening for TB or current chest x-ray
- Must possess an LPN or RN license in good standing with the State of Colorado

QMAP Caregiver Littleton

In this important role, you will coordinate prescription orders with doctor’s offices; oversee the central storage, tracking and delivery of medications as well as being responsible for the overall safety and health of all residents.

You will also be responsible for coordinating departmental schedule to ensure adequate staffing in accordance with legal requirements.

Requirements

You must maintain a safe and secure environment for all staff, residents, and guests following established safety standards, policies and procedures and maintain an inventory of supplies necessary for residential care.

You must be a certified RN, LVN or have 2 years of college in the human services or healthcare field.
You must have a current first aid certification
You must have a minimum of 2 years supervisory experience
You must be able to assist in the moving of residents when necessary

Under the supervision of the Manager of ADHC Services, the Day Center Health Care Tech (CNA) is responsible for providing and assisting participants with activities of daily living, restorative and supportive care. Essential Functions and Work Duties

PARTICIPANT CARE
• Assists drivers with the loading and unloading of participants from vehicles.
• Assists participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, hand and toileting as assigned or needed. Documents care provided on a daily basis on both appropriate paperwork and EMR.
• Participates in the care planning of participants through cooperation with the multidisciplinary teams.
• Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed.
• Monitors participants’ health status by taking vital signs and weights (monthly). Monitors appetite in addition to looking for changes in functional ability and checking skin condition while assisting with toileting. Documents findings in EMR and reports to the clinic as needed.
• Maintains the orderliness and cleanliness of the day rooms, kitchen and personal care areas.
• Monitors/assists participants in a courteous manner with meals and provides assistance with feeding as necessary.
• Assists participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed.
• Assists the transportation department with participants’ take home supplies.
• May rotate into In-Home Services as a Homecare C.N.A. as applicable and deemed necessary by the ADHC Services Manager.

Other Responsibilities
• Demonstrates a commitment to the quality improvement process and the philosophy of continuous improvement; identifies and responds actively and with sensitivity to the needs of all concerned; participates as a team player in all phases of the organization; and is open and responsive to change.

Education, Experience, Knowledge, Skills and Abilities:

• Must have a minimum three years of supervision and management experience in an assisted living facility or geriatric nursing environment.

• Computer skills required.
• Excellent communication (written and verbal) and interpersonal skills are required.
• Ability to effectively handle sensitive issues and maintain confidentiality.
• Ability to effectively lead a team.



Work Schedule: This is a full time day position Monday – Friday, with on-call responsibilities as needed.

Program Manager

Colorado Recovery, Inc. is seeking candidates for the position of Balsam House Program Manager. This position oversees all operations of Balsam House, Colorado Recovery’s residential treatment facility located in Boulder, CO, that provides services to adults with serious mental illness.

General Duties and Responsibilities:

• Oversee hiring, training, credentialing and supervising all regular and on-call environmental/residential workers.
• Supervise Balsam House therapist/case manager.
• Create and maintain a treatment environment that promotes healing, consumer empowerment, stigma awareness issues, and assists clients in exploring recovery principles.
• On-call for staff to contact after hours &/or in an emergency, on a rotating basis.
• Coordinate and communicate with managers of other CR programs, i.e., transitional and outpatient. Help to maintain continuum of services between different programs.
• Ensure compliance with all Colorado State Department of Health regulations.
• Assist CEO as needed or requested in order to ensure the programmatic success of Balsam House as part of Colorado Recovery.

Education and Experience:

• M.A., M.S. or M.S.W. in Social Work, Clinical Psychology or Human Services field in addition to at least five years experience working with persons with emotional and/or mental health issues.
• LPC or LCSW required; Addiction Counseling Certification(CAC) preferred.
• Other relevant degrees/licenses include Psy.D., Ph.D., and R.N.
• Residential experience with adults with severe mental illness preferred.
